The following documents are needed to enroll a child in Ohio Public Schools:  birth certificate, immunization records, proof of residency, and custody documents - if applicable.  Please contact the registrar, Fran Hansen at 330-435-1152 with questions.  If you are not able to upload the required documents in Final Forms, please contract Fran to schedule a time to drop them off.
